rabtech the person who I think did this name was Remi Ouellette. He built this for some video or movie in Canada. He had a serious illness that caused him severe pain which led to committing suicide in his burning sling shot. He still has a Facebook page.

Rob is correct about Remi. He actually had assisted death planned through the Canadian government and then as the government seems to do they screwed the system up and wouldnt let him die peacefully so he took his own life in a tragic way. He was a very creative man

I was lucky enough to see it in Montreal a few years back. I never got a chance to meet Remi, but the guy seemed like a creative genius.

These were made for the (last?) transformer movie.
